Herbert M. 'Burt' Formby

Herbert M. "Burt" Formby, 87, of Crescent City, Florida passed away on January 17, 2016. He was born on December 13, 1928 in Jacksonville to parents George Formby and Lottie McGaha. After serving his country in the US Army Air Corps, Burt settled in Jacksonville entering a business venture with his brother lasting 30 years. He then moved to Texas but returned to Florida after 35 years. Mr. Formby was a born again Christian and enjoyed fishing and hunting.Mr. Formby was the 5th child of 6, all deceased now along with his parents. He is survived by a daughter, Linda J. (Joe) Glisson; a son, Herbert (Ann) Peeples and numerous grandchildren, great grandchildren and great great grandchildren.A graveside service with military honors will be held Monday January 25, 2016 at 1:30 pm in Jacksonville National Cemetery, 4083 Lannie Rd, Jacksonville, FL 32218.
