Mr. Michael Mike Thomas Fallatik 52 of Keystone Heights died on Friday, May 6, 2011 from injuries he sustained in an automobile accident. Mike was born in Long Island, New York on April 1, 1959. He was employed with CMC in Starke as a Punch Hole Operator. Mike loved his family, the outdoors and music. He was also an avid Dallas Cowboy fan. His mother Millie preceded in him in death. Survivors are his wife of 25 years Pamela, children, Samantha Leigh Fallatik, Dallas Fallatik, and Rachel Johnson all of Keystone Heights. He is also survived by his father Vincent Cappicotta of Liberty, NY, and siblings, Alice Dows of Tucson, AZ, Marilyn Fallatik of Liberty, NY, Janet Lee of Plantation, Janet Fallatik of Sarasota, and Eugene Fallatik of Far Rockaway, NY.
